Water Testing Request

The H2O Quality Lab offers both tap water testing and well water testing. Please fill in the request below to the best of your ability. If you are unsure if you need testing, please refer to the FAQ. After submission, a member of the lab staff will reach out to you to confirm your request and provide you with payment instructions. Testing results will be released once full payment is received. Laboratory business hours:

Monday-Thursday: 7:30 a.m. - 1:30 p.m.

Friday: 7:30 a.m. - 9:30 a.m.

A $35 sampling and collection fee is added to all laboratory requests.

Commercial Customers Only: Chain of Custody

Commercial customers should download and complete the Chain of Custody form. This form must accompany all samples requiring testing.
